The facility is responsible for paying for the surveys to be conducted by a third-party vendor.

Facilities will need to enter the average scores for each CoreQ item provided by the vendor into the App; vendors will not have access to the App.

Surveys may be administered in different ways (i.e., by mail, by phone, etc.). We ask that you indicate how surveys were administered in your facility by checking any boxes that apply in field 5f on the Metric 5 data page of the App.

No. This measure is intended for resident opinions only ; family members may not complete the survey as a proxy for any resident.

We recognize this will be the case for some residents, especially those with cognitive impairment, who will not be able to share their views using the current system and questions. Just knowing that residents could not complete the survey is also good data for us to have, and this will be indicated by the number of responses facilities record.

In order to protect confidentiality of residents and to maintain a fair measurement process, facilities with fewer than 10 responses will not have results individually reported. (Their results will, however, be used to calculate statewide and regional averages.) It is crucially important that every facility is honest about their results. If fewer than 10 residents are able to complete the survey, that is important for us to know so we can make improvements in the process for next year.
